This form must be filled out, signed, and returned to Malarkey Roofing Products® at malarkey.technicalinquiries@amrize.com or mailed to the address below. Receipt of transfer fee of $500.00 will initiate the inspection and transfer process. All warranty transfer procedures, as found in the warranty, are to be followed in order for the warranty to be transferred.
